To protect your facility, your staff, and the youth you serve, we require one quick but important step:

A director or authorized decision-maker must review and sign the Program License Agreement before purchasing.
This agreement ensures:
  • ethical and responsible implementation
  • clarity around the non-clinical nature of the program
  • safety expectations and crisis boundaries
  • proper use of assessments and materials
  • compliance with your facilityā€™s supervision and reporting protocols
  • single-site licensing and intellectual property protection

Why This Step Matters

The Self-Worth Cultivationā„¢ program is an educational, non-clinical curriculum that supports self-worth, emotional regulation, and confidence in adolescent girls.

Because this program is implemented inside regulated environments such as:

  • psychiatric residential programs
  • behavioral health settings
  • juvenile and group-care programs
  • school-based or community therapeutic environments

ā€¦it is essential that leadership understands and agrees to the scope, boundaries, and proper use of this curriculum.

Your signature confirms that:
  • you understand your facility is responsible for all supervision and crisis response
  • you acknowledge this program is not treatment, therapy, or clinical intervention
  • you will ensure staff are trained using Module 0 before facilitation
  • your team will follow facility protocols for safety, reporting, and documentation
  • program materials will remain within the purchasing site

This protects everyone involved.

Step 1: Review These Documents

Please download and review the following required documents:

1. Program License Agreement
Outlines terms of use, licensing, intellectual property, disclaimers, and safety expectations.
2. Program Disclaimer and Scope of Use
Clarifies the non-clinical, educational nature of the program.
3. Legal + Safety Packet for Facility Onboarding
Covers boundaries, assessment usage, facilitator responsibilities, and implementation safety.
4. Crisis Response Boundaries
Defines what facilitators must do if a youth becomes distressed, unsafe, or in crisis.

These documents must be reviewed before signing.

Step 2: Sign the Agreement Below

Please complete the required signature form embedded below. This signature must be completed by:

  • the Program Director
  • Clinical Director
  • Administrator
  • or another authorized decision-maker

Once signed, you will automatically be redirected to the purchase page.

Step 3: Continue to Checkout

After the agreement is signed:

  • You will be directed to the secure checkout page
  • You will confirm agreement via a final checkbox
  • You can then complete purchase and gain access to Module 0
Important:
No program content will be accessible until the agreement is signed.

Implementation for Your Team

After purchase, Module 0 will guide your staff through:

  • Required safety and legal orientation
  • Facilitator responsibilities
  • Document review
  • Signing their Orientation Acknowledgment Form
  • Completing the Mandatory Compliance Quiz

Your facilitators must complete Module 0 before leading any groups.

I understand that all facilitators must complete Module 0, sign their Orientation Acknowledgment Form, and pass the Mandatory Compliance Quiz before leading any sessions. I understand that program materials are licensed for single-site use only and may not be shared, copied, uploaded, or distributed outside our facility. I understand that no outcomes or clinical results are guaranteed. I agree to all terms and accept responsibility for proper implementation at my site.
